Upload clear photos of: 1) Your Passport (preferably your Biometric passport but if not then your Ukrainian passport). 2) Any recent exit stamps from Ukraine in your passport. 3) For anybody under 18 who is not travelling with both parents, a note from any parent that is not travelling stating that they give permission for their child to travel to the UK under the Homes for Ukraine Scheme. 4) For anybody 18 or over, proof that they were living in Ukraine recently (within the last 9 months). For example a bank statement or utility bill or COVID vaccination certificate. 5) For anybody under 18, their birth certificate.
